    Default Gundersen Flap in severe corneal edema video

    Bullous keratopathy refers to severe edema of the cornea in which the epithelium is separated from Bowman's membrane by fluid. When the bullae containing the fluid rupture through the surface epithelium, the patient complains of severe pain, photopho bia, lacrimation and decreased vision. The eye appears red and irritated.

    Such episodes are relieved by regeneration of the epithelium but tend to recur frequently, so that pain may be nearly continuous. Bullous keratopathy is not rare. It may be associated with endothelial dystrophy ; chemi cal, physical or surgical damage to the corneal endothelium; adherence of the lens, hyaloid membrane, or iris to the back of the cornea ; epithelization of the anterior chamber; or long-standing glaucoma. The condition has been treated by local instillations of hypertonic solutions or oint ments to reduce the edema, and various lubricant drops and medicaments. No perma nent beneficial results have been obtained from these means. Bandages, sedatives, local anesthetics and medication for the control of infection and inflammation are usually employed. Keratoplasty has generally been of no avail. Filatov and Kalfa1 used a Denig buccal mucous membrane graft to relieve pain.
