     Short message✅

 Glaucomatocyclitic crisis ( Posner- schlozman syndrome )

    What u have to know

    ✍️ rare( I have faced 3 cases in 10 yrs )

    ✍️ unilateral

    ✍️ recurrent

    ✍️ middle age

✍️ very high IOP in relatively quite open angle eye ( asymtomatic just eye discomfort or very mild injection)

✍️ Herps viral trabeculitis is reported pathophysiogically ( herps simplex, CMV)

✍️ Topical steroids is mandatory ttt with dramatic response (main treatment)

✍️ short term ocular antihypertensive drugs during acute phase ( avoid PGs)

✍️ relatively benign disease with rarely reported glaucomatous optic neuropathy requiring TRAB

 Fuch's heterochromic Uveitis and Glaucomatocyclitic crisis

What is similar and What is different ..!??

    Similar in

    ✍️ rarity

    ✍️ unilaterality

✍️ high IOP in quite open angle eye

    ✍️ middle age group

✍️ viral iridotrabeculocylitis is pathophysiologically reported

    What is different ..!??

✍️ rubella virus is accused in FHUS ( unlike GCC, herps and CMV)

✍️ Heterochromia iridis ( best seen in daylight)
    • In FUHS is uniform unlike GCC patchy
    • regarding heterochromia: in dark iridis patient the lighter color is the diseased eye and the opposite is true )

✍️ satellite diffusely scattered kps all over the endothelium

✍️ iris atrophy moth-eaten unlike GCC patchy

✍️ angle twinglike NV( may case amsler hage during phacoemulsication due to suddenly dropped AC Pressure on entry )

✍️ usually associated with PSC cataract

✍️ although its inflammatory, steroids not necessary ( unlike GCC)

✍️ long term anti glaucoma medication or even more aggressive surgery is frequently reported ( unlike GCC)

    Enjoy all my dears 
