    Lack of pupil involvement in 3rd nerve palsy doesn't rule out an aneurysm specially in the sitting of early or partial 3rd palsy so MRI is mandatory particularly in younger age group

    If surgery is indicated
    It must be in the following sequence

    • orbital decompression
    • strabismus surgery if ocular misalignment
    • lid surgery if lid retraction

    That's because orbital decompression can solve all the problems mentioned or alter the ocular alignment and lid position

    Also strabismus surgery alone can solve problems related to both ocular misalignment and lid position

    If no recovery from Bell's palsy within 6 wks , u have to reconsider ur diagnosis as 10 % of acute facial palsy has treatable lesions

    Default RD after cataract surgery in children

    A 5.5% risk for RD is estimated for the first 10 years after cataract surgery in children with no known ocular and systemic anomalies. The risk significantly increases in a male, myopic, and intellectual disabled child.

    We emphasize the need for regular and long-term follow-up after pediatric cataract surgery.

    Proton pump inhibitors abuse( omiprazole and others ) increases the risk of formed visual hallucinations ( Charles Bonne syndrome) in moderate or severe visual loss in vAMD particularly in old age group ( with good mentality, differentiated from Alzheimer's disease)

    The incidence of retinal tears or retinal detachment after neodymium:YAG capsulotomy

    It is found <1% ( almost 0.5% )incidence of retinal tears or detachments occurring within the first year, with the greatest risk occurring in the first 5 months. Additionally, risks were different depending on patient age, with the greatest risk in the 61- to 70-year-old group.

    This study indicates that the risk of RD is much lower than currently taught in textbooks and residency training programs. Based on the results, as part of informed consent to perform a YAG capsulotomy, I now counsel patients that the risk of RD from the procedure is 1 in 200.
