Premenstrual Tension Syndrome Definition

= Ovarian Cycle Syndrome =Exaggerated Menstrual Molimina : a collection of physical and emotional symptoms related to a woman’s menstrual cycle.

Premenstrual Tension Syndrome Incidence:

*more common between 30-40 years.
*occurs only in ovulatory cycles.

Premenstrual Tension Syndrome Aetiology:

1) Endocrinal theory:
a) Salt and water retention due to: *↑estrogen/progesterone ratio.
* ↑ADH & ↑aldosterone.
b) Allergic reaction to corpus luteum.
c) Temporary ↑ in prolactin level due to stress factor & ↓PIF.
2) Diet theory: ↑salt intake & ↓vitamins A, B, E.
3) PGs theory: ↑PGs level during luteal phase.
4) Endorphins theory: ↓ β-endorphins level during luteal phase.
5) Serotonin theory: (low level depression & high level anxiety).
6) Psychological disturbances.

Premenstrual Tension Syndrome Clinical Picture:

A. Symptoms:
1) Nervous: headache, irritability, depression & aggression.
2)GIT: nausea, vomiting, diarrhea or constipation.
3)Fluid retention: edema and weight gain.
4)Mastalgia: breast swelling and tenderness.

B. Signs: done to exclude organic causes of the symptoms.

Premenstrual Tension Syndrome Treatment :

1) Reassurance.
2) Endocrinal theory:
a) Progesterone: antagonizes estrogen.
b) Diuretics (spironolactone):↓bloating sensation.
c) Anti-histaminic.
d) Bromocryptine (vitamin B6 is added to assist dopamine synthesis).
3) Diet theory: * Salt restriction. *Vitamins A, B & E.
4) PGs theory: anti-PGs e.g. mefenamic acid.
5) Endorphin theory: exercise ↑β-endorphin level.
6) Serotonin theory: *antidepressants for depression. *sedatives for anxiety.
7) Psychotherapy.
