Core Pulmonale

Definition It is a right ventricular ++ due to parenchymal lung
disease, vascular lung disease or chest wall disease
with or without failure.
Cause
1- parenchymal lung diseases Hypoxia.
Chronic obstructive lung diseases. (chronic bronchitis -
empysema)
b. Interstitial lung fibrosis
2- Vascular lung diseases
a. Core pulmonale
b. Thromboembolic P++ (subacute core Pulmonale)
Chest wall diseases e.g. Kyphoscliosis
C/P
1- Cause
2- Right ventricular++, pulmonary hypertension (see cardiomegaly)
Right ventricular failure S.V.C (see heart failure).
Investigations
ECG may be normal with emphysema or voltage.
Echo more accurate to diagnose Rt. V ++.
Treatment
Cause
Treatment of Rt. vent. Failure Diuretics, vasodilators.
Aminophylline
Digitalis (minimal role), you
can give small dose as there
is ­­ incidence of digitalis
toxicity
O2 therapy.
