This 7 years old girl is noted by her parents to have out deviation of her eyes when she looks up. She has no specific complaints but does say she can see double at times. Her general health is good and she is doing satisfactory work in school . Apart from this motility disorder her eyes' examination reveals no significant data
















This is a typical case of Brown syndrome. Brown syndrome is the inability to elevate the adducted eye , this is the only motility disorder of this girl .Of course, this must be confirmed by demonstrating restricted forced ductions to elevation of the left eye in adduction. Based on the typical appearance, it is safe to schedule surgery with confidence that the diagnosis of Brown will be confirmed. The choice of surgical procedure depends on the experience and the preference of the surgeon.

My choice is to do a medial approach to the superior oblique tendon and place a silicone spacer to lengthen the tendon.
